Funding Available
Momentum Learning Services is a registered provider under the Federal Government's 'Helping Children with Autism' package and most services (excluding professional training workshops).
Under this package, the Department of Families, Housing, Community Services and Indigenous Affairs (FaHCSIA) are distributing funding to families whose child has been diagnosed with an Autism Spectrum Disorder and is under seven years of age.

FaHCSIA Form Downloads
Clients using FaHCSIA funding to access our services need to complete FaHCSIA forms A, B and C when they first register for the service. Please contact us by phone or email and we will email or post you the forms or download them from the links below.
Once completed, these can be faxed or emailed in to Momentum Learning Services, but original signed forms must then be posted in as well. This is a FaHCSIA requirement.
Please note you must 1st make contact with an Autism Advisor in your state to identify whether or not you are eligible for funding before using these forms. This is confirmed when you have received a letter of introduction from FaHCSIA to verify this approval.
